The evaluation itself remains confidential; only summary recommendations are shared externally when required, preserving the participant’s privacy and intent of the work.
What It Includes
Interview, record review, and tailored measures
The process integrates a licensed clinical interview, relevant record review, and individually selected standardized measures chosen to match the referral context.
These tools identify strengths, challenges, and specific focus areas for growth.
All findings remain part of a confidential clinical record.
If external documentation is ever requested, only concise recommendations are shared—protecting the participant’s insights and personal information while meeting verification standards.

Court-Directed Recommendations
Professional summary for legal or professional use
When external reporting is required, the evaluator provides a concise, licensed clinical summary that addresses the referral question—such as treatment level, education track, or verification of participation.
Only the recommendations derived from the evaluation are shared; personal test data and private clinical content are never released.
Each summary is written in clear, defensible language that professionals can trust while maintaining participant confidentiality and dignity.
Participant Recommendations
Personalized plan for growth and accountability
Every participant receives individualized feedback written in plain, practical language—highlighting key strengths, skills to develop, and specific habits to monitor.
Recommendations connect insight to action, offering a structured framework for continued progress.
When appropriate, participants may be invited to enroll in a Change Tracks learning track or other supportive service aligned with their goals.
These recommendations transform evaluation findings into an actionable plan for growth—providing both direction and measurable follow-through.
Psychological Testing & Behavioral Measurement
Testing Tools
Evidence-based measures selected for the participant
Change Tracks uses validated psychological instruments to evaluate emotional regulation, stress tolerance, impulsivity, decision-making, and substance-use patterns.
Selection is individualized—not formulaic—so results are meaningful to the participant and relevant to the purpose of the evaluation.
Testing is completed securely online and interpreted directly by the licensed clinician who integrates results with interview and behavioral findings.
This ensures every measure contributes to a coherent, participant-centered understanding rather than a checklist of scores.
Measured Outcomes
Baseline → Midpoint → Discharge (verified change)
When applicable, select measures may be repeated internally to track change over time—clarifying growth in awareness, decision-making, and emotional regulation.
These results remain part of the confidential clinical record and are never released without the participant’s written consent.
If verification is requested, the evaluator provides only confirmation of participation and completion.
When a participant authorizes additional disclosure, brief summaries may be shared to support legal or professional coordination—never raw scores or private test data.
Evaluations are scheduled directly with each participant, with coordination provided for any professional referrals as needed.
